  • Preferred Provider Organizations (PPOs). These health plans assign you to a network of physicians and hospitals and also assign you a primary care physician. If you see physicians or specialists outside your network you may have to pay a deductible or pay the difference between what a network physician would charge and what the non-network physician charges . Co-payments are usually $5 to $10, and the PPO pays your medical bills.

  • These plans cost more than HMOs, but are cheaper than indemnity plans.

  • Point of Service Plans (POSs). These plans assign you to a network of physicians and hospitals, but you're allowed to use physicians and specialists outside the network by paying an additional fee. They put more emphasis on preventive care, such as offering health workshops and discounts to health clubs.

  • These plans cost more than HMOs and PPOs, but are cheaper than indemnity plans.
